Commissioner of Oaths
The Clerks Department provides Commissioner of Oaths services for the City of Woodstock. We reserve the right to refuse commissioning services of any document
Documents we may commission
- Consent letter for travelling with a child (commonly referred to as a travel document);
- Delayed birth registration forms;
- Election to change of name of child under the age of 12;
- Legal name change application;
- Foreign pension documents (or proof of life);
- Lost passport;
- Affidavit for a Family Gift of Used Motor Vehicle
Documents we cannot commission
- wills, "living wills", codicils to wills or estate settlement documents;
- divorce, separate, cohabitation and marriage agreements;
- custody documents;
- documents related to debt or finances;
- real estate-related documents, statements and transfer of land;
- court, legal or civil issue-related documents;
- any document that is to be executed by an individual acting in their capacity as a Power of Attorney or Executor
A commissioner has authority only to administer oaths and take affidavits. Office staff will not sign documents as witnesses.
Requirements
Once you are certain that the City of Woodstock can commission your document, you must visit City Hall in person.
- The request for the Commissioner of Oaths service must be made by the same individual taking the oath and signing the document.
- Documents to be commissioned must be filled out prior to arriving at City Hall and be unsigned:
- One (1) valid piece of government-issued photo identification (no photocopies) for each individual who will be making a declaration (i.e. a valid driver's licence, a valid passport or a Ontario Photo Card)
- If commissioning your document requires verification of your address please bring proof of address.
- If your identification is in a language other than English or French, an official translation of the document may be required.
Notary Services or Certified Copies
A commissioner of oaths is not the same as a notary public. We do not provide notary services. If you need the signature of a notary, please visit the province's website. The Clerk/Deputy Clerk may complete Certified True Copies, but they are not guaranteed to be accepted. Fees
The City of Woodstock charges $20 for each signature. This fee is payable by cash, cheque, or debit.
